Ewha Womans University Seoul Hospital succeeded in simultaneous surgery of Da Vinci SP'Colon cancer and liver metastatic cancer' for the first time in Korea

Jul 05, 2024

On the 20th of last month, a team of surgeons Noh Kyung-tae (colon anastomotic surgery) and surgical Cho Young-soo (concussion pancreas surgery) at Ewha Womans University Seoul Hospital successfully performed surgery for rectal and liver metastatic cancer at the same time using the Da Vinci SP robot system.

Simultaneous surgery involving anatomical resection in liver metastatic cancer is the first case in Korea, and Ewha Womans University Seoul Hospital reaffirmed the reputation of a single-hole robotic surgery institution through the conduction of medical staff with extensive experience in Da Vinci SP surgery.

Mr. A, in his 40s, visited Ewha Womans University Seoul Hospital because he had bloody stool again in January this year after first discovering bloody stool in October last year. The hospital confirmed rectal cancer after colonoscopy, and metastasis to the left lateral region of the liver was confirmed during further examination.

For the treatment of rectal cancer and liver metastasis cancer, specialists from the departments of gastroenterology, hematologic oncology, surgery, nuclear medicine, radiology, and radiological oncology at Ewha Womans University Seoul Hospital performed multidisciplinary treatment together, and after the 6th round of chemotherapy, simultaneous surgery was decided for rectal and liver metastatic cancer.

The surgery is a Da Vinci SP robot system, in which a robotic surgical instrument is inserted through an incision in the navel, and an additional penetrating needle is inserted into the part of the abdominal right ostomy so that the auxiliary instrument can be used.

First, Professor Noh Kyung-tae resected the rectal and lower S-shaped colon, including the tumor, along with the peri-tumor lymph nodes, and additionally performed resection surgery for the mesenteric, aortic, and pelvic lateral lymph nodes of the small intestine where lymph node metastasis was suspected.

Afterwards, Professor Cho Young-soo took over the surgery, completed the hepatic resection, confirmed that there were no abnormal findings such as bleeding or bile leakage, and handed over the surgery to Professor Noh Kyung-tae again.

Professor Noh Kyung-tae completed the surgery by connecting the remaining S-shaped colon with the rectum and making a stoma at the place where the additional penetrating needle was inserted.

Liver resection is at risk of massive bleeding during or after surgery, requiring considerable experience in liver resection and minimally invasive surgery such as laparoscopic and robotic surgery. In particular, only a few cases of anatomical resection of the liver using the Da Vinci SP system have been reported worldwide, and Ewha Womans University Seoul Hospital is the first case in Korea to perform anatomical hepatic resection at the same time as colon cancer surgery.

A was discharged healthy without complications after surgery, and continuous management and treatment will be performed through outpatient treatment.

Professor Cho Young-soo of Surgery at Ewha Womans University Seoul Hospital said, `It was possible because of active support and the hard work of many people at the medical center, and it became the basis for new attempts by learning the various skills and know-how of various senior doctors through academic conferences and papers.'

Professor Noh Kyung-tae of surgery at Ewha Womans University Seoul Hospital said, "In the past, when performing simultaneous surgery for colon and liver metastatic cancer, even if a patient wanted to undergo robotic surgery, he had to perform laparoscopic surgery due to technical difficulties in liver resection. Through this successful operation, we were able to expand the indications for robotic surgery, especially single-hole robotic surgery using Da Vinci SP."," he said.

Meanwhile, colorectal cancer is a malignant tumor that occurs in the colon and rectum and is divided into colon and rectal cancer according to the location of the occurrence. Daehak Seoul Hospital's colorectal surgery using the Da Vinci SP robot began for the first time in Korea and has performed more than 230 surgeries so far, playing a leading role in the field of single-hole robot surgery in Korea.
