Safety calibration with ICL lens insertion for high myopia and high astigmatism

Jul 10, 2024

Recently, my ophthalmology consultation office has received a lot of inquiries about ICL lens insertion. This is because lens insertion surgery is rapidly popularized, and people who want vision correction are highly interested.

When correcting vision, it is common to undergo laser surgery such as LASIK, LASEK, and more recently SMILE LASIK. However, if you have high myopia and high astigmatism, or if the corneal shape is too thin and asymmetric, ICL lens insertion may be almost the only way to remove the glasses. It is a surgery in which a small transparent lens of a certain frequency is inserted between the iris and the lens. Technology is greatly improved, so you can achieve good eyesight safely and effectively.

If you have high myopia, there are restrictions even if you want to get vision correction with LASIK, LASEK, or SMILE LASIK. This is because of corneal safety. You need to cut more corneas than regular myopia patients to get the desired vision.

In particular, in order to laser-correct the shape of the cornea that was crushed due to astigmatism, the cornea needs to be shaved 20-30% more than when correcting myopia only. If the amount of corneal shearing increases, the power to withstand intraocular pressure is weak, and corneal dilatation may occur.

Some nearsighted patients have severe astigmatism. At this time, a special lens developed for astigmatism correction called Toric ICL??Yes, it can be corrected. It is a small incision without suture, so recovery is fast. However, the correction effect of toric ICL decreases when the lens rotates when rubbing or touching the eyes after surgery, and sometimes requires surgery again. If astigmatism is severe, you have to order the lenses separately according to your personal frequency, which takes a considerable amount of time.

There is a useful way to completely relieve restrictions and worries.

It is a combination surgery that solves astigmatism and myopia at the same time by inserting a general ICL lens during astigmatism correction surgery. You don't have to use expensive toric lenses. Astigmatism correction surgery is a surgery to correct the crushed corneal shape with a fine knife of about 2.8 to 5.7 mm using an astigmatism tracking navigation device called 'Calisto Eye'. The principle of adjusting the tensile force of the cornea and correcting the shape when performing corneal transplantation is the core of astigmatism correction. Instead of cutting the cornea with a laser, astigmatism was corrected with surgical treatment to ensure its effectiveness and safety.

My ophthalmologist has published the results of correcting vision in this way in the Journal of the Korean Ophthalmology Society. Among the myopia patients who were unable to perform corneal refraction surgery due to high myopia or thin corneas, 16 patients with severe astigmatism were subjected to astigmatism and general lens insertion surgery at the same time, and the astigmatism frequency was significantly reduced from 2.98 diopters before surgery to 0.72 diopters. In addition, the naked eye vision of all patients who underwent surgery recovered to 0.8 or higher, and 81.25% of them showed 1.0 or higher.

In this way, astigmatism and general ICL combination surgery have better correction accuracy than when corrected only with an astigmatism correction lens. In addition, by reducing the astigmatism in advance using astigmatism correction, the thickness of the lens entering the eye becomes thinner, and the cost burden is relatively reduced as well as unnecessary time consumption.
