◇'Bile storage warehouse'Stone, gallstones in gallbladder

The gallbladder, located below the liver, is also commonly called the gallbladder, and is an organ that concentrates and stores bile and secretes it into the duodenum to help digest and absorb fat. However, when bile clumps and solidifies due to factors that can cause changes in the ratio of bile components such as old age, obesity, pregnancy, oral contraceptives, long-term fasting, severe diet, and metabolic diseases, gallstones occur. When gallstones cause inflammation of the gallbladder mucosa, cholecystitis can occur.

Cholecystitis can cause various symptoms such as pain in the solar plexus or right upper abdomen, high fever, chills, nausea, and vomiting. Even mild patients with mild pain may progress to sepsis and be admitted to the intensive care unit in extreme cases, so it is important to diagnose and treat them quickly. If cholecystitis is diagnosed through tests such as abdominal ultrasound and computed tomography (CT), surgical cholecystectomy is performed after calming the inflammation through antibiotic treatment, fasting, and fluid supplementation.



If the cholecystitis is left intact without surgery, it can recur later even if the cholecystitis is alleviated, so once diagnosed as cholecystitis, it is a principle to perform cholecystectomy. The gallbladder is not an organ that makes bile, but an organ that stores it, and bile is drained even after cholecystectomy, so it is not common for problems with digestive function to occur. Advances in surgery enable the resection of the gallbladder with laparoscopic surgery and robotic surgery with less bleeding and pain without laparotomy without laparotomy.

Gallbladder cancer, a tumor that develops in the gallbladder, ranks 8th in cancer incidence in Korea and has a poor prognosis with a 5-year survival rate of 26.7%. However, there are many cases where there are no symptoms due to cancer, and the wall of the gallbladder is very thin at 1-2mm, so cancer can easily spread to surrounding organs, so most of them are found to be advanced.

Depending on the degree of invasion of the gallbladder wall, the scope of surgery varies from simple cholecystectomy to gallbladder, liver resection, and lymph node excision, and chemotherapy may also be required. Gallbladder cancer is a cancer whose survival rate varies greatly depending on the timing of detection, and it is important to detect it early through a medical examination. In fact, most gallbladder cancer that can be operated is often accidentally detected without symptoms. Even when non-specific symptoms such as abdominal bloating and digestive disorders appear, it is necessary to confirm through active tests.

◇Improvement of living and eating habits is important...Need active medical checkups

Professor Shin Il-sang asked for the management of lifestyle habits and active health checkups to prevent various gallbladder diseases.

It is recommended to maintain a balanced and healthy diet, including a Korean food diet that evenly eats three to four side dishes, fiber-rich vegetables and fruits, and lean protein and unsaturated fats. It is recommended to avoid eating caffeine, soda, fatty meat, and fried food, and irregular eating habits such as binge drinking and binge eating should also be avoided.

Professor Shin Il-sang stressed that efforts are needed to detect gallbladder disease early through medical examinations to check one's health status along with such lifestyle correction, and it is important to identify the cause and treat it correctly through active treatment and examination even when digestive abnormalities such as simple abdominal bloating or digestive disorders appear.
