ASCEND Announces Global Agreements for Safe Use of Botulinum Toxin



The 'Aesthetic Council for Ethical Use of Neurotoxin Delivery (ASCEND)' held a press conference in Hanoi, Vietnam, on the 7th under the theme of 'Exploration of botulinum toxin resistance in the Asia-Pacific region: New Insights and Implications' and announced a global agreement for safe use of botulinum toxin.

ASCEND is a panel of international multidisciplinary experts in dermatology, plastic surgery, immunology and medical ethics, and is continuously conducting academic discussions to raise social awareness of botulinum toxin-resistant side effects.

In 2022, the International Society for Aesthetic Plastic Surgery (IMCAS Asia 2022) published the first paper on and urged medical staff to raise awareness of the potential risk of botulinum toxin resistance and to deliver accurate information.




The Asia-Pacific consumer survey was conducted from June to August 2024 on 2,588 consumers in nine Asia-Pacific regions, including Australia, Hong Kong, Indonesia, Malaysia, the Philippines, Singapore, Korea, Taiwan, and Thailand, and the medical staff survey was conducted from August to September 2024 and data was collected from 242 medical staff in eight Asia-Pacific regions, including Australia, Hong Kong, Indonesia, Philippines, Singapore, Korea, Taiwan, and Thailand.

Experts in various fields such as plastic surgery, dermatology, immunology, and medical ethics from various countries, including Korea, participated in the ASCEND press conference as a panel. The panel announced a second global agreement for safe botulinum toxin use with a paper on botulinum toxin resistance and safety on botulinum toxin resistance and safety.

At the same time, the ASCEND panel shared best practices for cosmetic and therapeutic toxin use and discussed ethical considerations surrounding them based on the results of a survey of medical staff and consumers in Asia-Pacific regarding botulinum toxin resistance.

According to the Asia-Pacific Consumer Survey conducted from June to August 2024, the percentage of consumers (n=2,588) who experienced reduced botulinum toxin treatment effects was still high at 81%. This is an increase from 69% in 2018 and 79% in 2021, confirming that the number of suspected botulinum toxin-resistant patients at home and abroad is increasing as the decrease in botulinum toxin effect is a major symptom that can confirm whether resistance occurs.

In addition, about 50% of consumers surveyed were aware of the potential cause of the reduction in botulinum toxin treatment effectiveness, but despite this recognition, 66% showed behavior patterns that could induce resistance, such as increasing the dose of the procedure, continuing treatment with products containing impurities, or shortening the procedure interval.

Dr. Niamh Corduff, the head of the meeting, said "The results of the survey show that many consumers are not fully aware that their behavior can lead to botulinum toxin resistance and reduce the effectiveness of the procedure, such as undergoing high-dose or short-cycle procedures."One of the main ways to prevent this is by using pure botulinum toxin preparations. With a wide range of product options available to botulinum toxin consumers, it should be emphasized that highly purified toxin preparations are important in accurately communicating product-specific differences and minimizing immunogenic risks" he said.




During the discussion, ASCEND panels emphasized that the role of medical staff is of paramount importance because it is difficult for patients to fully understand the causes and potential effects of botulinum toxin resistance without clear guidance from medical staff.

Park Je-young, CEO of Apgujeong Oracle Dermatology Department, said "The risk of developing resistance is increasing as the number of high-dose and high-frequency procedures of botulinum toxin has recently increased in Korea. Medical staff should understand the history and purpose of the procedure in detail through systematic consultation with patients and guide patients on safe procedures options that minimize the risk of resistance"By preventing resistance, patients can enjoy a long-term botulinum toxin effect. In particular, this is important in that toxin is used not only for cosmetic purposes but also for treating various diseases."
