Seoul's Seonam Hospital to Prepare for Super-Aged Society 'Seonam-type Medical and Care Integration System'

The Seonam Hospital of Seoul City will promote the first 'Seonam Medical and Care Integration System' project of Seoul City Hospital for the elderly in Korea, who are about to enter the super-aged society, to continue living in healthy communities.

To this end, a meeting to expand primary medical institutions in the southwestern part of Seoul will be held on the 27th to gain momentum for the integration of the southwestern medical and care system and seek cooperation for the leading direction of development.

Developed countries that have experienced super-aged societies before us are implementing community integrated care policies to help them live a healthy old age where they lived, and Korea is also attempting various medical care policies such as chronic disease management, door-to-door treatment, home medical center, depression treatment support, and multi-pharmaceutical management projects to realize sustainable medical care.




Cho Young-gyu, Director of the Public Medical Division of Seonam Hospital, said, `Integrated care in the community is completed through integration, centered on people rather than the disease itself, and not separation of medical institutions and communities.'"By establishing an integrated medical and care system for Seonam Hospital, Seonam Hospital will play the role of a representative public medical institution in the southwestern part of Seoul by cooperating with primary medical institutions in the region so that the elderly can continue to live in the community and have a healthy retirement." he said.

Clinics and nursing homes, which are primary medical institutions in the region participating in the construction project, will be provided with ▲ 24-hour emergency room hotlines for rapid emergency patient transfer ▲ Integrated nursing and nursing wards for requested patients ▲ Establishment of discharge plans for discharged patients ▲ Comprehensive regional health care services such as re-request (medical referral) to primary medical institutions for discharged patients.

In addition, it plans to provide comprehensive cooperation and support programs such as infection control and emergency response education to establish a safe medical environment for primary medical institutions such as nurses, physical therapists, and social workers belonging to primary medical institutions.
