Flight crew crash on runway ahead of takeoff, how?

According to local media such as The Sun, a female flight attendant crashed on a low-cost British airline TUI Airlines flight scheduled to take off from East Midlands International Airport at around 4:30 p.m. on the 16th (local time).
The flight attendant was found to have opened the aircraft door as the plane was preparing to take off.
It was estimated that he stepped on the door thinking that there would still be stairs.
Falling on the runway, she was taken to the hospital by ambulance and emergency helicopter.
The Aviation Accident Investigation Bureau (AAIB) and the airline are conducting further investigations.
